Registration Form	This form should be printed, filled out and returned to the KOC office.
Emergency Information	This form should be printed, filled out and returned to the KOC office.
Contract Agreement	This form should be printed, filled out and returned to the KOC office.
Statement of Consent	This form should be printed, filled out and returned to the KOC office.
Contract Addendum Custody Issues	This form should be printed, filled out and returned to the KOC office only if parents are no longer together and they share custody or there are custody issues.
Credit Card Authorization	This form should be printed, filled out and mailed or turned in to the KOC office if you would like to automatically charge your credit card each month for regular tuition.
Fee Information	This form should be printed, filled out and returned to the KOC office.
Contract	This gives you important information about Kids On Campus fees and parent responsibilities.
Site Addresses	This document includes the center addresses and phone numbers as well as KOC office address, phone and fax numbers.
Parent Handbook	This explains KOC policies and procedures.
Center Information	This tells you what we offer at each of our sites.
Childcare Fees	This explains our fees and how you are charged.
Important Dates	This tells you what days KOC is available and what days we are closed.
Calendar	This is a colored calendar that shows camps, holidays and other closures.
