Affidavit Forms (As needed) - NEW AND CURRENT STUDENTS

• Affidavit of Residence - To be completed only if parent/guardian/caregiver and student are living with another family.
• Afidávit de Residencia - To be completed only if parent/guardian/caregiver and student are living with another family.
• Affidavit of Parent/Guardian for Proof of Age of Minor - To be completed only if parent/guardian is unable to provide proof of age documentation.
• Caregiver's Authorization Affidavit - To be completed if the parent or legal guardian is not the party enrolling the minor child in school. The signing of this affidavit is sufficient to authorize enrollment of a minor in school and authorize school-related medical care.
