Certificate of Insurance Request

A Certificate of Insurance (COI) is a document that verifies an individual or business has insurance coverage.  It is issued by an insurance company and summarizes key information from an existing insurance policy, such as coverage limits, expiration dates, and various types of insurance coverage. 

If you are in need of a Certificate of Insurance, please prepare the required documents prior to submitting your request. 

Reasons for requesting a COI:

  1. You are initiating a Contract agreement for services (such as providing a service for students) with a 3rd party that requires a certificate of insurance. 
  2. You are interested in taking students on a field trip to various locations, and the facility is requesting that you provide a certificate of insurance. 
  3. You are taking students to a 3rd party location (overnight included), and the location is requesting a certificate of insurance through your contract agreement. 
  4. You are applying for a grant, and the contract agreement requires a certificate of insurance in order to issue any funding. 
  5. You have a contract with a 3rd party, and they are supplying employment to the students enrolled in your program.  The contract requires you to provide proof of insurance and workers’ compensation coverage for student workers. 

These are a few examples of why you may be requesting a certificate of insurance.  

Please review the “HOW-TO Request a Certificate of Insurance” document.  Please submit your request along with all required documents and allow up to 3 weeks to process.  These requests are processed in the order they are received.
